Service Plan Information

I want to add more hours to my service plan.

Any changes to your service plan requires approval from your Person Centered Counselor/Counselor/Support Coordinator. If you need their contact information please contact Customer Service via phone or chat. We are happy to help!

FEIN Verification

A FEIN is needed for all employers.

  1. Dial 1-800-829-4933
  2. For English- Press 1 or Spanish- Press 2
  3. For Employer Identification Number Information- Press 1
    • How to apply for FEIN- Press 1
    • Received a reference number- Press 2
    • If you already have a FEIN but forgot it or need a confirmation letter- Press 3
    • Recently applied for an EIN via fax and didn’t receive confirmation- Press 4

If you already have a FEIN, our employer training department needs a copy of the letter to schedule your visit.
